ASSUMPTION OF RISK
I acknowledge that participation in the sporting activity of hockey involves personal risk to the participants, including a risk associated with possible negligence or breach of legal duty by others. On behalf of myself I, my heirs, accept and assume all such risk.
WAIVER OF LIABILITY
In consideration for my participation in the SWHL, SWANSEA WOMEN’S HOCKEY LEAGUE, I on behalf of myself, my family members, executors, administrators, successors and assigns, hereby agree to remise release and forever discharge, waive and save harmless, protest, and keep indemnified ‘SWHL’, George Bell Arena, Lampton and/or its staff, its affiliated corporations, sanctioning bodies, participants, competitors, entrants and all respective agents, volunteers, officials and servants and representatives from and against any and all kind of actions, claims, cost and expenses and demands in respect to death, injury, loss or damage to my person or property however caused arising out of my being permitted to attend at or in any way take prior to, during or subsequent to this league whether as a spectator, participant, competitor, entrant or otherwise and notwithstanding that the same may have been contributed to or occasioned by the negligence of any of the aforementioned, their agents, officials, servants or representatives.
I AM UNDERSTANDING THE ASSUMPTION OF RISK AND WAIVER OF LIABILTY PROVISIONS SET OUT ABOVE AND AGREE THAT BY SIGNING BELOW I AM ACCPETING THE TERMS THEREOF AND WAIVING LEGAL RIGHTS WHICH, I, MY FAMILY OR LEGAL REPRESENTATIVES MAY OTHERWISE HAVE.
